Mentorship Registration/Questionnaire

Mentorship is a committed partnership between two disciples of Jesus--where one is a little farther in the journey than the other (this does not mean that age is what makes us farther on the journey).  
Entering into a mentorship relationship requires time, commitment, and a desire to grow in trust, openness and vulnerability with each other.  
Meeting with a mentor occurs monthly for approximately 1-2 hours.
